To the Honorable City Council:

 

The Mayor respectfully requests that this Council accept and approve expending a grant of $237,939.81 from the Metropolitan Area Planning Council to the Health and Human Services Department under the provisions of MGL Chapter 44, Section 53A. This grant does not require the commitment of new matching funds.

 

The purpose of this grant is to support a team including a Shared Services Coordinator, a Regional nurse, and a Clinical Community Health Worker to perform work on behalf of Somerville as part of the multi-municipal shared public health services initiative. This work includes assisting in the collection of data with municipal health departments, the assessment of local public health systems, and engagement with key community stakeholders, health care, and human service-based and community-based organizations.
